Access Care Centre is a registered NDIS provider based in Brabham. We support people with disabilities, their families and carers across Perth’s north and south-east, at home and out in the community.
Our support workers help with personal care, toileting, medication, meals, mobility and household tasks, so participants can stay safe and comfortable in their own homes. See in-home care and daily personal activities. For complex needs we offer high intensity daily personal activities and community nursing care.
Community access support helps participants take part in social, recreational and community activities, with transport where it’s in their plan.
Our Level 1 and Level 2 support coordinators help participants understand their plan, connect with providers and manage their budget. Capacity building supports help people build everyday skills for more independence.
