Assistance in coordinating or managing life stages/transitions and supports
To help participants live at home and engage in their community, we provide both short-term and long-term supports that are centered on enhancing their capacity to manage their supports. consists of:
A support connection
Assistance with housing and tenancy duties
Support coordination
Support with daily planning, financial management, and mentoring
assistance with life transition planning, including peer support, individual skill development, and guidance
Development of daily living and life skills
The goal of developing daily living and life skills is to help the person or their carer live as independently as possible. This includes providing supports that will make it easier for the person to travel and use public transportation on their own. For instance, how to use public transport.
Plan Management
Plan management involves overseeing the financial aspects of a participant's supports, including making payments to providers, processing expense claims, issuing monthly statements, and submitting claims for payment to the NDIA.
