We provide Level 1 and Level 2 Support Coordination services to participants, working one-on-one to help them:
Understand and effectively use their NDIS plan to achieve their goals
Connect with NDIS providers, community supports, and other relevant services
Build their confidence and skills to manage and coordinate their supports independently
Understand, monitor, and manage their NDIS plan budget
Level 1, Support Connection: short-term help to connect you with community, mainstream and NDIS-funded supports, so you can start using your plan with confidence.
Level 2, Coordination of Supports: ongoing help to understand your plan, choose and connect with providers, sort out problems as they come up and get ready for your plan review.
We can also help with housing and tenancy tasks, daily planning and money management, and planning for big life changes.
